Consent to Video and Audio Recording of Classes

For the purposes of teacher training, professional development, and quality control and improvement, SMU and its instructors affiliated with the Mindfulness Initiative @ SMU may at times want to video and/or audio record course sessions.

I understand that any visual recordings will focus on the course instructor and not show the faces of participants. I understand that there may be an audio recording of the class discussion but that I cannot be identified individually (e.g., by full name) other than by my voice. I understand that the video/audio recording will only be used for training, certification, and quality-related purposes and will be held confidentially.

Assumption of Risk and Undertaking

Assumption of Risks: I understand that participation in this course carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, from person to person, and include both physical and psychological risks, which are rare especially for serious risks. Potential physical risks of physical exercises include injuries such as scratches, bruises, sprains, joint or back injuries, and concussions. Potential psychological risks of mindfulness practices and meditation include increased negative affect, heightened anxiety, loss of a sense of meaning, unveiling and re-emergence of traumatic memories, and depression.

I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in this course. I assert that my participation is voluntary and that I knowingly assume all such risks. I also commit to notifying the course instructor as soon as I suspect experiencing serious side-effects of this course.

Undertaking: I assume full responsibility for all risks associated with my participation in the course and I hereby irrevocably undertake to release, waive, discharge and covenant not to sue, to the fullest extent permitted by the laws of Singapore, SMU, its staff, officers, faculty, or any of its authorised agents in respect of any and all claims, liability, demands and causes of action of whatever kind that I may have including, but not limited to, any damage to or loss of any personal property or illness or personal injury or death or the consequences thereof, arising from or in any way connected with my participation in the course, where such damage to or loss of any personal property, or illness or personal injury or death, or the consequences thereof is not caused by the negligence of SMU, its staff, officers, faculty, or any of its authorised agents.
